Year 2024 Q3
Type CMS + CRM · LMS · Analytics
JCCSP (Jockey Club Carer Space Project) is a Jockey Club–funded smart platform for elderly carers — bringing the public website, self-help e-coaching & e-learning, centre activities and a full case-management system into one place. Horizon Tech built an integrated platform that replaced five disjointed systems, with 100+ operations staff across centres and 12,000+ carers and members assessed.

BRAND WEBSITE

A warm, public home for carers

The face of the project, jccarerspace.hk, gives Hong Kong’s elderly carers a calm, bilingual place to land — centre activities, the latest news and a curated resource centre, all designed to feel welcoming rather than clinical.

E-LEARNING

Caregiving skills, turned into a learning playground

The e-learning platform reframes carer training as a gamified theme park. Themed booths — loving yourself, the community, and caring for a loved one across communication, housing, mobility, food and medical needs — take carers through bite-sized, level-by-level lessons they actually enjoy finishing.

E-COACHING

Self-help coaching, set on a friendly island

The e-coaching experience turns evidence-based coaching into an explorable archipelago. Each island — healthy living, mood care, warmth at home, restful sleep and more — hosts interactive scenes and characters that walk carers through real situations at their own pace, any time they need support.

CARER JOURNEY MAP

Every carer’s story, mapped

The Carer Journey Map turns a carer’s lived experience into a visual timeline — roles, challenges, turning points and personal needs laid out along a single winding path — helping carers reflect on how far they’ve come, and helping the team meet them where they are.

CASE MANAGEMENT SYSTEM

One source of truth for 100+ frontline staff

Behind the scenes, an integrated case-management system replaced five disjointed tools — connecting admin, centre operations and assessment so 100+ staff across centres work from the same records, and 12,000+ carers and members have been assessed without data slipping through the cracks.

ANALYTICS · METABASE

Self-serve insight, no analyst required

A Metabase-powered report builder lets operations teams answer their own questions on the fly — live need-hierarchy and utilisation insights that turn everyday service data into decisions, backed by a written performance-improvement plan.

RESULT

Five systems, now one platform

JCCSP brought a fragmented landscape into a single, governed platform: carers find what they need in seconds, and live insight helps the Jockey Club and centres direct support to where it matters most.
